United States Immigration Examination Instructions
-- You will need to make at least two visits to our office. To minimize the likelihood that you will have to come back more than twice to our office, please make sure to comply with all the following requests.
-- Please bring any official immunizations records you have and make sure that these are translated into English if necessary otherwise there will be a charge to accomplish this.
-- Please bring a picture ID (driver’s license or passport).
-- Please bring any bottles of medications you are taking.
-- Please bring an interpreter if you have trouble speaking or understanding spoken or written English.
-- Please bring Form I693 (found at (click here)) with the required sections filled out to your appointment.
-- The exam includes a general medical examination and a tuberculosis blood test for ANYONE over the age of 2 (including pregnant women). Blood tests for syphilis is required for those 18-44 years old and a urine test for gonorrhea for 18-24 years old.
-- Any required vaccines can be obtained at our office, with your own physician and/or at a local health department for additional fees. There are blood tests for immunity that can be performed in our office only in place of any missing vaccination documentation
--The fee for the examination is based on age and any blood tests performed or vaccines provided.
--If one’s tuberculosis test is abnormal, a chest x-ray is required and is done at another certified facility. This x-ray will be a separate fee paid directly to the x-ray facility.
