United States Immigration Examination Instructions
-- You will need to make at least two visits to our office. One will be with the physician and one with a nurse and these visits must be spaced 2-3 days apart. To minimize the likelihood that you will have to come back more than twice to our office, your first appointment should not be scheduled on a Thursday or prior to a long holiday.
-- Please bring any official immunizations records you have and make sure that these are translated into English if necessary otherwise there will be a charge to accomplish this.
-- Please bring a picture ID (driver’s license or passport).
-- Please bring any bottles of medications you are taking.
-- Please bring an interpreter if you have trouble speaking or understanding spoken or written English.
-- Please bring Form I693 (found at (click here)
-- The exam includes a general medical examination, a tuberculosis skin test for ANYONE over the age of 2 (including pregnant women) which must be read 48 to 72 hours later in the 2nd office visit and a syphilis blood test for those 15 years or older .
-- Any required vaccines can be obtained at our office, with your own physician and/or at a local health department for additional fees.
-- A chest x-ray is required for adults and is done at another certified facility. This x-ray will be a separate fee and is payable directly to the x-ray facility.
